Description

Abstract: Precision medicine is becoming more pervasive and points to a more holistic approach to individualized patient care. We are conducting a series of studies to understand how to best administer a social determinants of health questionnaire prior to their incorporation in electronic health records (EHRs). Here, we explore in-person administration of a hybrid social determinants of health instrument in a community clinic to gain insight into data collection methods and guide standards for EHR implementation.

Learning Objective 1: Learn about social determinants of health and their core function in precision medicine, as well as their potential use and applicability into electronic health records.
