Abstract: Lack of timely follow up on abnormal test results is common and can have unfortunate consequences. We examined the safety culture of a tertiary academic medical center surrounding test results in the ambulatory setting using resources commonly used for quality improvement. Using these sources of data synergistically allowed us to examine multiple perspectives in test result safety culture and aids in identification of explanations and solutions for inappropriate follow-up.

Learning Objective 2: Identify common reasons for inappropriate or untimely test result follow up.
